UFO Check-List Incident Summaries 1–100
This file is a compilation of standardized 'Check-List – Unidentified Flying Objects' forms documenting UFO sightings, predominantly from the summer of 1947. According to the releasing agency (U.S. Department of War), each summary contains structured details about an incident, and many also include witness lists, statements, and narrative reports. The forms capture observer identity and occupation, location and date of observation, distance, direction of flight, time in sight, weather, and physical descriptions (construction, exhaust trails, odor, effect on clouds). Visible entries include a 9 July 1947 sighting by Major Richard R. (a test pilot) at Muroc AAField, California; a Portland City Police Department report by Sergeant Claude describing three discs in echelon; a 30 July 1947 report from Pan American Airways at Harmon Field; and sightings near Boise and Mountain Home, Twin Falls, Lockbourne AAB, Hamilton Field, and between Medford, Oregon and Mt. Shasta. Documents bear Confidential and Secret classification markings with declassification authority NND 917033. Individual claims are observer accounts and are recorded here as reported, not verified.
Description
A 209-page declassified U.S. Department of War compilation of 'Check-List – Unidentified Flying Objects' incident report forms, numbered roughly 1 through 100. Each entry records observer names, occupations, observation locations, dates, object descriptions, time in sight, and weather, with many including witness statements. Released via war.gov's UFO/UAP disclosure release.
Claims
Numerous observers, including military and police personnel, reported disc-like unidentified flying objects, predominantly during July 1947.

A Pan American Airways station mechanic reported an object leaving a bluish streak.

Observation reported aboard a flight between Medford, Oregon and Mt. Shasta.
